Please send your intake form and the agreed supporting documents at least five days before your FCA appointment. Your booking instructions should show the due date and explain how to use the secure upload process.
The OT reviews this material before the interview. Tell us early when a document will be delayed or when the form is difficult to complete.
Complete the intake form
Use the client ID and form sent with your booking information. The current form is a fillable PDF intended for a computer; save your progress as you go.
Answer from your experience. Describe the assistance you normally use, including reminders, equipment and help from other people. Explain when an activity varies rather than choosing only your easiest or most difficult day.
It is fine to use notes or to have someone help you communicate. Make clear which information is yours and which has been added by another person. Where a questionnaire has its own instructions or timeframe, follow those instructions and flag anything that does not make sense.
Contact us before the deadline if the format is inaccessible. We need to discuss an alternative that suits the assessment and confirm any change to scope or cost before you proceed.
Choose the documents that answer the assessment question
Useful documents may include a current NDIS plan, treating reports, previous assessments or records of a significant change. For a child, information from school or an early childhood setting may be relevant. Not every person needs every type of document.
Start with material that explains the reason for this assessment. Older reports can provide history, while current records may be needed to describe present needs. When there is a large volume, ask which documents to prioritise rather than sending an entire file without explanation.
Use the secure upload instructions provided. If you later find information that could affect the assessment, contact us before uploading more. We need to decide how it should be considered and whether timing needs to change.
Plan for the appointment
The interview is usually about an hour and takes place by video only. You need a screen and a working video connection. Check the device and connection beforehand and, where possible, choose somewhere you can speak privately.
Tell us beforehand about communication aids, a support person, fatigue or any other adjustment. Keep your notes nearby. You do not need to demonstrate an activity that is unsafe or stop using your usual assistance.
The session is recorded so the person on the call can listen rather than type. If being recorded worries you, say so when you book.
If you need to pause during the interview, say so. If you cannot attend, contact Seeds promptly and check the cancellation terms in your agreement.
