One upload Everything comes to us in one go
We begin preparing your assessment the moment your upload arrives, so this is the one chance to include everything. Anything that comes later can't be added to it.
We need it all at least five days before your appointment. Your form and your documents are read in full before we meet you, and the interview is shaped around what we have read — five days is what that takes.
Before you begin, please have
- your intake form finished and saved
- every report, letter or assessment you would like considered
- all of it saved on this device, ready to choose
- each file under 40 MB, and 90 MB across everything — a very large scan may need to be sent at a smaller size
- at least five clear days before your appointment

What should I prepare before starting?
If they are available, documents that show what daily life looks like over time help the assessment land — but don't stress if you can't get them. We can still complete an FCA without them.
Most helpful, if available
GP health summary · NDIS plan · specialist letters · therapy reports (OT, psychology, speech, physio, behaviour support) · school, workplace or community reports
Helpful, but not essential
Hospital or clinic discharge summaries · imaging or investigation results · a medication list, with side effects · support-worker or carer notes · previous assessments such as WHODAS, COPM or ABAS-3
There is room for your intake form plus up to 12 supporting documents. If you have more than 12, combine related reports into one PDF, or send the most recent of each kind — recent and relevant beats everything.
Your intake form
The PDF you filled in, saved with your answers still in the boxes. We open the file when it arrives and read the answers straight out of it, so a printout, a photo, a scan or a "print to PDF" copy is turned away: those turn your answers into a picture we can't read.
So we can read your answers
- Open the form in Adobe Acrobat Reader (free) on a computer. Phone apps and some web browsers lose the answers when they save.
- Type your answers straight into the boxes.
- Save with File then Save, or the save icon. Do not use Print, "Save as PDF" or "Microsoft Print to PDF": printing flattens the form and we can't read it.
- Check it: close the file, open it again and click a box. Your answers should still be there, and still editable.
- Choose that saved file here.
Page 2 of the form says the same. If a box on the form was too small for what you had to say, don't worry: every box scrolls and nothing you typed is lost, as long as the file is saved rather than printed.
